This committee makes decisions that determine the formulary status of medications.
How does the three-tier copayment benefit work?
Under the three-tier copayment benefit, all eligible products are covered as follows: Tier One (lowest copayment) covers preferred generic products; Tier Two (middle copayment) covers preferred brands; Tier Three (highest copayment) covers non-preferred brand and generic products.
